Cross-border workers and newcomers: the 3 months that decide everything

Right of option and affiliation deadline: a wrong choice can cost you for years.

Updated 5 September 2026 · Information drawn from official legal texts (fedlex.admin.ch), the Federal Office of Public Health (FOPH) and Swiss federal data.

Newcomers to Switzerland

You have 3 months from taking residence to join a health insurer. Cover takes effect retroactively from your arrival date, and so do premiums. So don't wait: you pay the same either way, but you choose your insurer instead of being assigned one by the canton (often an expensive one).

Cross-border workers: the right of option

Depending on your country of residence, you can choose between Swiss LAMal cover and your home country's system. The choice must be made within 3 months of starting work and is essentially irrevocable.

  • Cross-border LAMal: access to care in both countries, premium independent of income.
  • Home-country system: contributions often income-based, sometimes cheaper on low incomes, much more expensive on high ones.
Warning: never let the deadline pass without choosing, the rarely optimal default assignment becomes your regime. Get both options priced before signing anything.
