| Premium year | Average increase |
|---|---|
| 2020 | +0.2% |
| 2021 | +0.5% |
| 2022 | nearly flat (0.2%) |
| 2023 | +6.6% |

Why do premiums keep rising?
FOPH attributes the increase mainly to the growing number of treatments, population ageing and medical progress. Basic insurance costs rose by 5.3% in 2025 and are expected to grow by 5.0% in 2027.
- More treatments: the volume of care per insured person keeps growing, especially in outpatient care.
- Billing still unclear: delays linked to the new TARDOC medical tariff and outpatient flat rates make the 2026 trend hard to read.
- Reserves barely used: five insurers priced their premiums as close as possible to expected costs and ten took their investment income into account, but none presented a plan for voluntarily reducing reserves.
Why does the national average say nothing about your premium?
Your premium depends on your insurer, canton, premium region, age, deductible and model. An average of +4% can mean +15% for you, or a decrease.
A Deloitte analysis of 2026 premiums shows a counter-intuitive effect: while the official average increase was 4.4%, the cheapest offers on the market rose by about 7%. Those who had already chosen the cheapest insurer often faced the biggest increase: switching last year is no reason not to compare again.
What can you do now to pay less in 2027?
- Wait for your insurer's letter (by end of October): it gives your real figure, not the national average.
- Compare using official 2027 data: our premium comparison shows the top 5 insurers for 2027 by canton, deductible and model.
- Adjust the two levers: deductible (CHF 300 to 2,500) and model (family doctor, HMO, telemedicine: up to 20% discount).
- Sign up with the new insurer before cancelling: for basic insurance, no insurer can refuse you.
- Cancel in writing, by registered mail or A-Post Plus as FOPH recommends, around 20-25 November.
